Ethics Code Verification

 All students in the Counseling Psychology program are expected to familiarize themselves with and adhere to the current ethical standards and code of conduct for psychologist. This form is used to provide verification to the program that our students have (a) received a copy of the current APA Ethical Standards and Code of Conduct for Psychologists, (b) have familiarized themselves with it, and (c) have been advised and encouraged to consult with their program advisors, faculty and supervisors on issues of ethical and professional concern.
 
This form must be signed and returned to the program coordinator or director within the first 6 weeks of students? first fall enrollment in the program. (For students who entered the program prior to Fall 2003, please sign and return the form within 2 weeks of receipt.)
 
My signature below indicates that I have received or obtained the current APA Ethical Standards and Code of Conduct for Psychologists and have been advised to asked questions of my program advisor, faculty or supervisors about any ethical issues or concerns that arise during my tenure in the program.
